Start post-exposure prophylaxis as soon as possible, ideally within 24 hours and no later than 72 hours after exposure. PEP is recommended where the source is known HIV positive with a detectable viral load, or of unknown status with risk factors. The standard course is 28 days of antiretroviral therapy.
